Vision benefits are provided by VSP (800) 877-7195. Members enrolled in a small group or individual and family plan have pediatric vision benefits through the end of the month in which they turn age 19.

Optional vision coverage is available for small and large group members when elected by the employer. Refer to the Sutter Health Plus Evidence of Coverage and Disclosure Form for information regarding your vision benefits.
