Dental benefits are provided by Delta Dental (800) 422-4234. Members enrolled in a small group or individual and family plan have pediatric dental benefits through the end of the month in which they turn age 19.
Optional dental coverage is available for small group members when elected by the employer. Refer to the Sutter Health Plus Evidence of Coverage and Disclosure Form for information regarding your dental benefits.
